What is HealthPathways?

HealthPathways Illawarra Shoalhaven (HPWIS)

  • an initiative for the Illawarra Shoalhaven Local Health District (ISLHD) and COORDINARE – South Eastern NSW PHN
  • a free web-based portal to support general practitioners and other health professionals in assessing, managing and referring patients.

PATHWAY DEVELOPMENT
310 pathways are now published. Click on localised pathways to access.

To track pathways under development click on PATHWAY PROGRESS in the top menu bar.

SITE USAGE
Site usage is growing as more pathways are localised. In comparing the 2016 and 2017 years between January and November the session usage has increased 43% whilst users have increased 15%.

To track monthly and quarterly usage click on the USAGE DATA in the top menu bar. Each month the usage is rising exponentially.  

The site has been live since 21 Sept 2015.

REFRESHED HOMEPAGE
A new look homepage is now published to ensure it:

  • is an easier site to navigate
  • is of similar design to other regions' homepages including a consistent table of contents layout
  • has space to promote health alerts
  • has capacity, in 2018, to activate mobile phone display compatability.

CLINICAL EDITOR OPPORTUNITY AND ACHIEVEMENTS
A part time GP Clinical Editor opportunity is available. Click on the previous link to access the advertisement.

Dr Louise Delaney has published a significant number of pathways for our local site since 2015, too many to name individually. Louise was recently appointed to also advise and support other HealthPathways GP Clinical Editors across NSW. 

Dr Bronwen Spalding has published many pathways including a chapter for palliative care, orthopaedic referral/request pathways for urgent, non-urgent and fracture clinic assessment and an Osteoporosis pathway.

Bronwen recently published a suite of mental health pathways including detailed provider directory information on communuity, public and private services. The clinical pathways published are: Bereavement, grief and loss, Anxiety in adults, Borderline personality disorders, ADHD in adults, Physical health and mental illness, Depression in adultsMedications for moderate or severe depression in adultsDeliberate self harm and Suicide risk. An LGBTI Support Services page was also published.  

Dr Annette Roberts has completed her assignment to localise a chapter of Older Persons' Health pathways.

CLINICAL ENGAGEMENT LEADER
Tony MacDermott is the Clinical Engagement Leader for HealthPathways in the Illawarra Shoalhaven. Tony connects with GP’s and other clinicians to build engagement with HealthPathways, from the development phase to product utilisation and feedback. Tony works in collaboration with the clinical editors and the HealthPathways team in the ISLHD to support and promote a resource which facilitates consistent and efficient care pathways for patients and clinicians alike.

Looking for clinical pathways and referral information:

Electronic Referrals
Behind the scenes, COORDINARE and ISLHD are gearing up to ensure the HealthPathways site in mid 2018 is the central portal to locate referral options and send secure electronic referrals using Best Practice or Medical Director. This is a much needed advancement that has been raised by many GPs at clinical working groups.
Provider Directory
As clinical pathways are localised in 2017 a new style provider directory is being collated to list public, private and other community options. This is a tremendous development for the Illawarra Shoalhaven. The site's search engine is an efficient way to locate a pathway, specialist or service.
Partnership Approach

The HPWIS Team appreciates and thanks the District Clinical Divisions and Departments for their advice. ISLHD Clinical Stream Directors and other specialists are providing oversight on clinical pathways.

Our GP Clinical Editors spend time with these clinicians to ensure pathways are localised expediently and with the latest medical assessment and management information. ISLHD Clinical Nursing and Allied Health professionals are also providing tremendous support to ensure the advice and request/referral pages and support information reflect current service provisions.

The COORDINARE and ISLHD partnership is ensuring GPs, specialists and other health professionals are working together with patients at the centre of the discussions and developments.
