What is HealthPathways?

HealthPathways Illawarra Shoalhaven (HPWIS)

  • an initiative for the Illawarra Shoalhaven Local Health District (ISLHD) and COORDINARE – South Eastern NSW PHN
  • a free web-based portal to support general practitioners and other health professionals in assessing, managing and referring patients.

To track pathways under development click on PATHWAY PROGRESS in the top menu bar.

The 10 most viewed pathways for 2017 are listed to the right within the Snapshot of site usage.  

Site usage is growing as more pathways are localised. In comparing the 2016 and 2017 years between January and September the session usage has increased by 35% whilst users have increased by 11.5%. 

Reports for past quarters are listed to the right of this content box. The site has been live since 21 Sept 2015.

Dr Louise Delaney has published a significant number of pathways for our local site since 2015 and is currently rolling out Child Health pathways. Louise was recently appointed to also advise and support other HealthPathways GP Clinical Editors across NSW. 

Dr Bronwen Spalding has also published many pathways including a chapter for palliative care.
Bronwen is publishing a suite of mental health pathways including detailed provider directory information on communuity, public and private services. The clinical pathways will be published once the service information suitably reflects recent provider changes in the mental health and related supporting community.
Bronwen is currently working with the Surgery team and has just published orthopaedic referral/request pathways for urgent, non-urgent and fracture clinic assessment. An Osteoporosis pathway has just been published.  

Dr Annette Roberts has completed her assignment to localise a chapter of Older Persons' Health pathways and these are on the live site.

As clinical pathways are localised in 2017 a new style provider directory is being collated to list public, private and other community options. This is a tremendous development for the Illawarra Shoalhaven. The site's search engine is an efficient way to locate a pathway, specialist or service.

Behind the scenes, COORDINARE and ISLHD are gearing up to ensure the HealthPathways site is the central portal to locate referral options and send secure electronic referrals using Best Practice or Medical Director. This is a much needed advancement that has been raised by many GPs at clinical working groups.

The HPWIS Team appreciates and thanks the District Clinical Divisions and Departments for their advice.

ISLHD Clinical Stream Directors and other specialists are providing oversight on clinical pathways. Our GP Clinical Editors spend time with these clinicians to ensure pathways are localised expediently and with the latest medical assessment and management information.

ISLHD Clinical Nursing and Allied Health professionals are also providing tremendous support to ensure the advice and request/referral pages and support information reflect current service provisions.

The COORDINARE and ISLHD partnership is ensuring GPs, specialists and other health professionals are working together with patients at the centre of the discussions and developments.

2017:     COPD and related chronic lung diseases CWGs to be held for the Illawarra and Shoalhaven in November.

              Lung Cancer CWGs held in the Shoalhaven and Illawarra
          Related pathways published with one on lung cancer-established due in August.

2016:     Mental Health CWGs held in the Illawarra and Shoalhaven - most referral/support pathways are published
              whilst the clinical ones will be published soon.
2015:     Palliative Care CWG held - pathways published
              Respiratory Services CWG held - pathways published
              Renal Services CWG held - pathways published with an evaluation underway.
